The Chelan County PUD has been fined $6,000 dollars for workplace safety violations related to death of employee Edward “Eddie” Bromiley in June.
The state Department of Labor and Industries found three violations during its five-month investigation into the death, including two that were deemed serious.
Bromiley was killed June 13 during crane testing at Rock Island Dam when he was struck by a steel component.
L&I said the PUD failed to properly warn workers about the potential hazards of a swing rail at the spillgate and the possibility it could become unhinged.
The PUD says it has corrected the problems and has contracted for a third-party investigation that is ongoing.
Bromiley is survived by his wife and two young children.
